You will read through the case study for the Unit 8 Assignment. Using this case study, you will:

  1. Evaluate models of behavior using the basic model of health promotion
    and disease prevention, explain in your own words the systemic process
    that contributes to your client’s healthy or unhealthy habits. Evaluate
    the mind-body connection between your client and their health.
  2. Make a list of your client’s needs, prioritizing them, and evaluate a plan for meeting these needs.
  3. Create a treatment plan for your client. A treatment plan typically
    includes a diagnosis of the problem based on the client’s history,
    treatment goals, and steps the client will take in order to achieve the
    treatment goals. Make sure your plan is specific, measurable, and
    achievable and that it contains timeframes for the goals to be achieved.
    Your paper should be 6–8 pages, and you should use at least four
    resources, other than your text, to research the disorder you chose for
    your Assignment. You must have at least one Kaplan Library resource for
    your Final Project, from library databases such as Academic Search
    Premier or Lexis Nexis, along with online articles and books.
  4. Information regarding APA formatting is at the Kaplan Writing Center.
    APA formatting dictates how your paper should appear on each page. Refer
    to the APA Quick Reference on the Course Home Page for examples of how
    to cite sources.

Each paper should include:

  • A title page
  • The paper itself (the “discussion”)
  • Correct APA formatting for in-paper cites
  • A references page
  • Use standard margins: 1″ on all sides.
  • Use standard 12-point font size in Times New Roman or Arial.
  • Use standard double-spacing: average of 22 lines per page, and between 20 to 24 lines per page.
  • Use left-aligned text. Do not right-justify.

John
goes to his doctor because he is having severe stomach problems. John
has a long history of other health issues that his doctor has not been
able to explain from a physical point of view. John’s doctor has decided
that it is time for him to have a complete physical. His doctor asks
him if there is anything else he should know. John tells his doctor that
all is fine.
John’s doctor is aware that about 8 years ago, John
attended and successfully completed an inpatient rehab for intravenous
heroin use. Sensing that John might be resistant to opening up with him,
his doctor asks him how his recovery is progressing. The doctor wonders
if John is eating OK and exercising, since he has lost a lot of weight.
John finally tells his doctor that his wife is ready to leave him, his
job is on the line, and he feels depressed. John admits that, as a
result of his personal problems, about six weeks ago, he relapsed with
alcohol, then with heroin, and he has used heroin on 3 other occasions
since then. He is also experiencing some flu-like symptoms and swollen
glands and worries that they have not gone away as he expected they
would. 
In reviewing the test results, the doctor determines that
John has alcoholic hepatitis. He tells John that he knows that there is a
major problem with the relapse, and he needs help; if he does not
change his lifestyle, he will die. The doctor makes an appointment at
the local Drug and Alcohol Treatment Center, which John agrees to keep,
however the doctor is concerned that John may not go.
