Interdisciplinary Management of Healthcare Technology: Permanent Pacemakers
The heart’s conduction system is vital in order to sustain life. Electrical impulses travel through the myocardium causing the heart to contract at a specific rate and rhythm, delivering blood throughout the body. With that being said, when the heart’s electrical system is not working properly, a permanent pacemaker may be needed. Permanent pacemakers are a therapeutic modality used to treat ill patients, such as in heart failure, or patients with complex needs to correct a slow or irregular heartbeat. P.L. is a 58-year-old male who had a history of recurrent episodes of syncope from sudden drops in heart rate and blood pressure due to vagus nerve sensitivity. The recurrent episodes of syncope were greatly affecting P.L..’s day-to-day life. After discussing treatment options with his health care provider (HCP), the conclusion of receiving a permanent pacemaker was made. The pacemaker which P.L. received detects contractions in the heart muscle which tend to occur before syncope episodes. When these contractions are detected, the pacemaker releases electrical signals preventing the sudden decreases in heart rate (“Pacemaker program,” 2017). This paper will discuss the explanation and background of permanent pacemakers, provide its risks and benefits, interdisciplinary team roles and responsibilities, nursing scope of practice, and patient education.
Explanation and Background
Cardiac pacing with the use of pacemakers has and continues to evolve. A cardiac pacemaker is an electronic device that is used when the normal conduction pathway of the heart is damaged, in which the device would function to pace the heart (Harding et al., 2020). With permanent pacemakers, the device is implanted within the body. The device consists of two parts: a pulse generator and wires (leads) (Verma & Knight, 2019). The number of leads may vary depending on the type of device used (i.e., single/dual-chamber or biventricular pacemaker). The leads function to sense cardiac activity, with connection to the myocardium, and deliver electrical impulses that stimulate the heart to beat (Verma & Knight, 2019).
A permanent pacemaker is not limited to a specific patient population. The device is, however, commonly associated with the elderly population. In part, this is due to the association of aging coupled with comorbidities that eventually warrant device implant (Walker, 2018). Despite this, a pacemaker can be implanted as early as the infancy stage of life. In addition to the population varying, device implant indications also vary. Given this, patients with pacemakers may have varying medication regimens. It is important to note which medications have the potential to affect the device’s performance, which may subsequently alter the pacing threshold. Specifically, antiarrhythmics can negatively affect the device function and anticoagulation therapy should be reviewed pre, peri, and post-operatively (Koski & Lay, 2016).
With evolving technology, some pacemakers are now designed to be MRI-safe, which for some, can make living with a pacemaker more adjustable. There are precautions to consider, however, such as refraining from magnets or their magnetic fields. Another safety issue to consider is the battery life on the device, which can be followed with routine device interrogations and reprogramming. Undergoing initial device implant and management with device interrogations can be costly, but insurance companies do offer coverage for most of these expenses, varying on an individual basis.
Risks and Benefits
With the insertion of a permanent pacemaker, there are many risks and benefits involved. The patient has the autonomy to decide whether the benefits outweigh the risks when considering proceeding with the implantation of the device. A pacemaker can benefit a patient as it regulates the heart’s rhythm. The patient needs to understand that a pacemaker will not prevent or “cure” heart disease or risk of myocardial infarction. It can, however, eliminate symptoms related to sick sinus syndrome, third-degree atrioventricular block, and bradycardia. Symptomatic bradycardia symptoms can include chest pain, shortness of breath, hypotension, and confusion (Harding et al., 2020).
Ways to promote positive outcomes include the administration of prophylactic IV antibiotics before the procedure, ensuring the correct device type is being inserted, which will vary depending upon the patient’s diagnosis, and providing the patient with all information needed to feel comfortable. This includes informing the patient what to expect before, during, and after the procedure. In doing so, the patient’s anxiety levels will likely be decreased.
On the contrary, risks, or complications, that may arise when undergoing insertion of a permanent pacemaker include infection and hematoma at the insertion site, pneumothorax, and perforation of the atrial or ventricular septum by the pacing lead (Harding et al., 2020). Further complications can also arise once the device has been implanted. This includes pacemaker malfunctions such as device failure to sense, capture, or pace and appearance of “end of life” battery power when testing the device (Harding et al., 2020). Although these complications could have a detrimental effect on the patient, some mechanisms can be implemented to prevent any of the above complications from occurring.
Considerations to implement for preventing complications include administering IV antibiotics prophylactically after device insertion, post-device insertion chest x-ray to ensure all leads are adequately placed and secured, visual observation of the device insertion site, and continuous ECG monitoring of the patient’s rhythm to ensure the device is sensing, capturing, and pacing as programmed (Harding et al., 2020). In addition, arm, shoulder, and range of motion activity limitations can be implemented post-operatively to prevent lead dislodgement. To ensure complications are prevented and the patient’s device is functioning as programmed, assistance from interdisciplinary teams is integral.
